Prescreen Form

If you prefer to prescreen your deal prior to application, download and complete the Prescreen form, then email the Prescreen form to one of our Business Development Officers (listed below)

Submit Application, forms and required items in one of three ways

1) Email completed form to your Business Development Officer (BDO).
2) Request your BDO to email a link or invite you to a folder, created specifically for your loan request. Then, drag and drop all required items into your folder, and your BDO will be notified immediately of your upload.
3) Request your BDO to email a link or invite you to a folder, created specifically for your loan request. Then, drag and drop all required items into your folder, and your BDO is notified, immediately, of your upload and has instant access.

Business Loan Application Package

This package is the application package and applies to the operating business, which is the borrowing entity. (Guarantor package below).

Guarantor Package

Any owner, stockholder, member owning 20% or more of the borrowing entity must complete a guarantor package.

Application (3 pages)

History of Business (1 page)

Schedule of Collateral (1 page)

Business Debt Schedule (1 page)

Source of Injection (1 page)

Accounts Receivable / Accounts Payable Aging Summary (1 page)

Third Party Contact Form (1 page)

IRS Form 4506T (1 page)

Working Capital Worksheet (1 page)

Environmental Questionnaire (required on real estate to be used as collateral)

Suggested Business Plan Outline (required on start-ups & expansions)

Projection Worksheet Year 1 Month-to-Month (required on start-ups and expansion of services and product lines) (if you already have your projections those are acceptable)

Projection Worksheet Year 2 & 3 Annual (required on start-ups and expansion of services and product lines) (if you already have your projections those are acceptable)
